  • Transitioning patients from an inpatient setting to stable outpatient care
  • Optimizing the management and treatment of heart failure through lifestyle modifications, oral medications, device therapy and aquapheresis
  • Reducing the risk of hospital readmission
  • Evaluating patients with severe heart failure and determining advanced surgical therapy options, including LVAD implantation or heart transplantation
